We require just a few items to register your child for RCYS sports.

1.    Registration Form (Online preferred-click here to register)

2.    $65 registration fee ($60 for additional siblings)
        You may pay online using PayPal (no account required)
        You may pay by check or cash at Evaluations
        You may mail a check in (prior to August 1st for early rate).

3.    Insurance/Medical Waiver (click here to complete and print)
       (If you don't print this now, you'll have to complete one at Evaluations.
        It will make it easier on you and speed you through the line if you
        complete the online form, print it out, and bring it with you.
