We require just a few items to register your child for RCYS sports.

1.    Registration Form (Online preferred-click here to register)

2.    $65 registration fee ($5 off for each additional sibling)
        Payments received after March 6th will require a $15 late fee per child/registration.
        You may pay by check or cash at Evaluations
        You may mail a check in (prior to March 3rd for early rate).

3.    Insurance/Medical Waiver (click here to complete and print)
       (If you don't print this now, you'll have to complete one at Evaluations. It will make it easier on you and speed you through the line if you complete the online form, print it out, and bring it with you.
